At the heart of the Evolution maternity solution is functionality that is designed specifically to provide support for the care of pregnant women, new mothers and their babies. As well as being an on-line system for data collection and management as part of the care delivery process, midwives will benefit from using Evolution particularly in the areas of auditing local clinical practice, generating statistical reports, avoidance of the need for duplication of data entry, access to summaries of care, and for research purposes. Managers also benefit from by using Evolution to assist in estimating future workload, reporting on actual activity and management of the allocation of resources

Solution Overview

Enhanced Safety and Workflow Efficiency
Evolution enables workflow to be both seamless and intuitive with built in levels of safety, convenience and efficiency at every stage of the pregnancy journey. Designed to reduce errors, maximise resources and promote an improved experience for women on the healthcare they receive.
Clinical alerts notify the user of pertinent data such as medical history, allergies, MRSA and HIV status or risk factors such as declined screening tests, smoking status, alcohol and drug misuse, domestic violence, child protection and mental health issues.
Rules-based decision support functionality interacts with clinical data to reduce the number of clinical incidents and support treatment decision making. Reviews of treatment given on a per condition basis with comparisons over time leads to improved management of conditions.

Clinical Noting
Evolution includes a wide range of clinical protocols for simple, quick and efficient clinical noting of all examinations. These include general health checks, gynaecological examinations and a wide range of antenatal, intrapartum and postnatal examinations of the mother and, where appropriate, the baby. Data entry can be completed by a variety of modes, including free text input or selection from pre-defined response lists.

Timeline for Maternity
Evolution records multiple instances of care events in a variety of clinical protocols from routine antenatal care through to specialist areas, such as diabetes clinic, HIV clinic and early pregnancy assessment. Data can be viewed from within a hierarchical structure of patient notes, patient-centric summaries, system generated reports or user-designed ad hoc reports. When the mother is initially referred for maternity services, a care plan is established which details the care the mother will receive. This care plan can be updated at any time to reflect changes in the patient’s care management. It is therefore possible to track the changes to the mother’s care.

Antenatal Care
Evolution has the facility to record the relevant current and past obstetric and clinical history, including all previous pregnancies. The maternity record is permanent and, once established, these details are available for view at all times. If successive pregnancies are managed through Evolution, details of these recorded past pregnancies remain accessible to users.
A qualitative summary of the results of blood pressure monitoring can be recorded into a variety of assessment forms at different stages throughout the pregnancy.
Results of foetal and maternal Doppler blood flows and CTGs can also be recorded at a variety of points in the pregnancy. A tabular view summarises the trend through the pregnancy.

Labour and Delivery
A detailed record of both staff and others present during the delivery is held, together with a record of the staff responsible for each patient intervention. A comprehensive record of both the actual and intended place of delivery, as well as a full and detailed record of the process of the labour and the delivery is kept. Evolution provides a real-time or retrospective entry of labour and delivery data.
Adverse outcomes of pregnancy, such as stillbirths, can be recorded, which can alter the subsequent behaviour of the system by preventing patient communications being sent inappropriately, such as letters, whilst ensuring that the care providers involved are being notified.

Postnatal Care
Evolution includes a full postnatal care record which includes the ability to record any maternal postnatal complications.
The postnatal examinations of mother and baby provide an opportunity for the recording of both their state of health and any public health advice given.
Easily accessible, accurate records of the prevalence of infection within the maternity units can be provided by taking a daily snapshot of current infections which allows for trends to be established and appropriate actions to be taken.
Supports the optimisation of nutrition by providing access to baby feeding regimes information which can also provide an in house evidence base for best practice

Data Analysis and Management Reporting
Evolution is equipped with a comprehensive portfolio of pre-written reports to comply with nationally mandated statistical reporting requirements. Examples include:  RCOG annual returns, Robson Category Audit, National Obstetric Anaesthetic Database (NOAD) and Korner returns.
The pre-written standard reports in conjunction with user definable adhoc reports can be used to highlight maternity caseloads, improving services for particular vulnerable groups, meeting targets for breastfeeding and decreasing smoking levels and identify areas of high level concerns such as Perinatal and infant mortality which is supplemented by extracting information from the database and populating a replica of the Perinatal Death Notification Form issued by the Centre for Maternal and Child Enquiries (CMACE)
Using the build it ad hoc report generator, information can also be collected to support planning for addition resources such as staff and/or equipment based on activity and utilisation and to create an easily accessible reflection of activity/occupancy of the maternity unit to support optimised use of the resources.
