Referral & Discharge Management

Our Electronic Correspondence Module (ECM) is a web based workflow solution designed to create, manage and deliver discharge summary information for patients discharged from an inpatient stay in a secondary care setting.

Summary

The solution provides workflow processing of discharge summaries and ensures that the appropriate users provide authorisation at key stages, with each user playing a specific role. This role may be assigned permissions to authorise specific elements of the overall discharge summary. The solution can be easily customised to suit local practice and is both flexible and intuitive to user needs with a user-friendly user interface and with individual views provided for different parts of the organisation.

ECM also:

  • supports multi-hospital environments, where secondary care establishments can deploy a single solution across multiple sites
  • is configurable to suit the needs of individual organisations – for example, data entry requirements for different wards, specialties or hospitals
  • facilitates the automatic generation and delivery of discharge summaries and other correspondence.
enables multiple secure methods of delivery to Primary Care.

Key Benefits

  • discharge summaries are compiled and delivered quickly and efficiently as part of the care process and with minimal effort and impact upon care providers
  • delivers legible and rich discharge summary information that covers all aspects of the patient’s stay in hospital
  • helps organisations move towards a paper-free environment, including the ordering of prescriptions for the patient To Take Away
  • holds a repository of archived discharge letters, which can be viewed by relevant staff and reported upon
  • provides a secure web based solution that can be quickly deployed across a site, with only a client web browser required for access
  • can connect to any patient administration, clinical or departmental system to utilise existing information and populate discharge summary letters automatically
  • saves time and effort and provides process traceability
  • meets the NHS Contract for Acute Hospitals targets (delivery of discharge information to GP practices within 24 hours)

Key Features

Workflow - enforces a clear workflow on the production of discharge summaries, whilst remaining sufficiently flexible to enable users to work efficiently. It ensures that all steps are completed and authorised before the discharge summary is released.

Authentication - user can be given permissions specific to their role, and different users may be granted different mixes of permissions, as required.

Templates - enables an organisation to customise data entry templates, even being able to specify different templates for different wards and/or specialties and/or hospitals (if the application is deployed across multiple sites).

Automatic Delivery of Output -  includes facilities to automatically deliver discharge summaries to recipients both internal and external to the trust. This ensures the discharge summary is delivered in a timely fashion and in a standardised readable format.  Transport mechanisms include:

  • Email via the Trust’s MAPI-compliant email system, such as Microsoft Exchange or Novell GroupWise
  • Email via NHSnet
  • Fax via NHSnet
  • DTS to GP practice systems such as EMIS, so that the application can deliver discharge summaries directly into the patient record, ready for viewing within the GP system

Customisation and Configuration - contains many items that are configurable or customisable to suit the needs of an organisation. For example, drug, diagnosis and treatment lists can be managed to ensure they meet the specific needs of users.

User Views -  the system is partitioned into separate user views that are published to different groups of users , e.g. ward nursing staff, pharmacists, consultants and clinicians, and systems administrators.
